Human Milk Banking Re-emerges

Reconstruction

Human milk banking turned an ancient practice—one lactating woman nourishing another woman’s infant—into an organized clinical service. The documented chronology, however, does not begin in Stockholm in 1919. The first generally recognized human milk bank opened in Vienna in 1909, followed by a service in Boston in 1910. In May 1919, pediatrician Marie-Elise Kayser established Germany’s first public milk-collection station in Magdeburg. Her program recruited donors beyond the hospital, collected surplus milk from their homes, subjected donors and milk to medical oversight, and distributed the preserved milk to infants whose mothers could not provide enough.

Stockholm followed a related but distinct path. At Sachsska Children’s Hospital, opened in the early 1910s, several resident mothers and wet nurses supplied fresh breast milk for hospitalized infants. When wet nurses became difficult to recruit in the late 1940s, a human-milk center was established at Södersjukhuset. According to a historical account of Swedish pediatrics, it supplied pasteurized human milk across the Stockholm area. This late-1940s institution, rather than a documented Stockholm foundation in 1919, provides the strongest local basis for the card.

The food being banked was expressed human milk, usually intended for premature, low-birth-weight, sick, or nutritionally vulnerable infants when their own mothers’ milk was unavailable. Early systems varied: milk might be expressed by hand or pump, collected in bottles, chilled, transported, tested, and boiled or pasteurized before controlled distribution. Later practice increasingly combined donor health histories, blood screening, hygienic expression, frozen storage, microbial testing, traceable containers, and standardized Holder pasteurization—heating milk to about 62.5°C for 30 minutes, followed by rapid cooling. Pasteurization improves microbiological safety but reduces some heat-sensitive biological activity.

Milk banking did not expand in a straight line. Improved commercial formula and changing hospital routines contributed to decline in many countries during the 1950s and 1960s. The HIV/AIDS crisis of the mid-1980s then caused additional closures because milk could transmit HIV if untreated and donors could not yet be screened as reliably or economically as they later were. Stronger testing, pasteurization standards, neonatal intensive care, and renewed evidence favoring human milk supported a resurgence around and especially after 2000.

The lasting significance is clinical rather than culinary. Compared with formula, donor human milk reduces the risk of necrotizing enterocolitis in preterm or low-birth-weight infants, although formula can produce faster short-term growth. Fortification, introduced commercially in the 1980s, helped address the high protein and mineral requirements of very premature infants. Modern Sweden retains a hospital-based milk-bank network, making donor milk a carefully governed bridge until a parent’s own milk becomes available—not a replacement for supporting parental lactation.

Historical context

Stockholm was a rapidly growing industrial capital with about 419,000 inhabitants in 1920. Europe was emerging from the First World War and the 1918–1920 influenza pandemic, while physicians and public-health reformers were confronting high infant mortality from infection, diarrhea, unsafe substitutes, and malnutrition. Refrigeration, sterilizable bottles, breast pumps, bacteriology, and expanding children’s hospitals made it increasingly possible to separate milk production from feeding. Stockholm already had milk-distribution charities and the Sachsska Children’s Hospital, but its region-wide pasteurized human-milk center is documented only from the late 1940s.
